Rename clipmenu.el to ivy-clipmenu.el
TL;DR: Preparing ivy-clipmenu for publishing. Also: - Removes lingering TODO items. - Clarifies module and function documentation. - Defines groups for custom variables. - Supports history variable for ivy-read.
